What is Liver Enlargement (Hepatomegaly)?
Liver enlargement (hepatomegaly) is an increase in liver size caused by many possible conditions — fatty liver (steatosis), inflammation (hepatitis), congestion, infections, storage disorders, or space-occupying lesions. Symptoms can include right upper abdominal discomfort, fullness, fatigue and abnormal liver tests.
Common causes (modern)
- Non-alcoholic fatty liver disease (NAFLD) / alcoholic liver disease
- Viral hepatitis, metabolic or storage diseases
- Drug-induced liver injury, congestive hepatopathy
- Inflammation, fibrosis/cirrhosis in advanced stages
Ayurvedic perspective
Ayurveda relates liver disorders to impaired Agni (digestive/metabolic fire) and accumulation of Ama (toxins), with Pitta imbalance affecting Rakta and Rasa. Treatment aims to correct Agni, remove Ama, pacify aggravated Pitta, and support liver tissue (Yakrit/pleeh) regeneration.
Essential Medical Testing & Safety
Before pursuing intensive Ayurvedic detox (Panchakarma) or internal medicines, baseline medical tests and imaging are essential:
- Complete liver function tests (ALT, AST, ALP, GGT, bilirubin)
- Viral hepatitis markers (HBsAg, HCV), metabolic profile (lipids, fasting glucose)
- Ultrasound abdomen (to assess liver size, steatosis, focal lesions)
- Coagulation profile and other tests as indicated
We coordinate with your physician — some conditions require urgent medical care and Panchakarma is contraindicated in acute hepatitis, decompensated liver disease, severe coagulopathy or infection.
Ayurvedic Strategy for Liver Enlargement
Our clinic follows a cautious, evidence-informed pathway:
- Medical clearance: Confirm safety through tests and imaging.
- Correct Agni & remove Ama: Digestive regulation using light diet, herbal bitters and panchakarma preparatory measures.
- Targeted detoxification: Virechana (therapeutic purgation) is the principal Panchakarma for Pitta/liver disorders under supervision.
- Supportive therapies: Abhyanga, Swedana, Kashaya Dhara, Njavara Kizhi and Oil Bath as adjuncts to improve circulation, reduce inflammation and nourish tissues.
- Herbal & nutritional support: Hepatoprotective herbs (Guduchi, Kalmegh/Andrographis, Bhumyamalaki, Turmeric), diet and lifestyle changes to prevent recurrence.
- Monitoring & follow-up: Repeat labs and imaging to assess response and safety.
Clinic Therapies Useful for Liver Enlargement (with photos)
Therapies below are commonly used in liver-supportive protocols at our clinic. Virechana is the key liver-directed Panchakarma; others are supportive to improve circulation, digestion and tissue nourishment.
Virechana (Therapeutic Purgation) Primary
What it is: A medically supervised purgation procedure that cleanses Pitta-related toxins via bowel evacuation after preparatory oleation and fomentation.
- Directly targets Pitta and Rakta imbalance — used for fatty liver, Pitta-predominant hepatomegaly and chronic inflammatory liver conditions (after medical clearance).
- Helps improve liver metabolism, reduce congestion and support reduction in liver size over time when combined with diet and herbs.
- Requires preparatory Snehapana (internal oleation) and Swedana; performed under physician supervision with monitoring of vitals and labs.
Abhyanga (Medicated Oil Massage)
What it is: Warm, full-body herbal oil massage tailored to the patient's constitution and liver condition.
- Improves peripheral and splanchnic circulation, aids lymphatic drainage and supports elimination of toxins.
- Prepares the body for Panchakarma (including Virechana) and reduces muscular tension and fatigue associated with liver disease.
- Used daily during preparatory phase and intermittently for maintenance.
Swedan (Herbal Steam / Fomentation)
What it is: Whole-body or localized herbal steam to induce sweating and mobilize toxins.
- Used as a preparatory therapy before Virechana — aids in liquefying Ama and mobilizing toxins toward the gut for elimination.
- Improves circulation to abdominal organs and reduces congestion.
- Carefully dosed when liver inflammation is present — performed under clinical guidance.
Kashaya Dhara (Herbal Decoction Pouring)
What it is: Continuous pouring of warm herbal decoction over the abdomen or upper abdomen to soothe tissues and reduce inflammation.
- Can be used cautiously to soothe hepatic region, improve local circulation and reduce abdominal congestion.
- Selection of decoction depends on Pitta balance and skin sensitivity; always done with medical oversight.
Njavara Kizhi (Nourishing Bolus Therapy)
What it is: Warm boluses of Njavara rice cooked in herbal decoction applied to the abdomen and body to nourish tissues.
- Provides deep nutritive support to soft tissues, promotes circulation and aids gentle tissue repair.
- Useful in the rejuvenation phase after detoxification to build vitality and support liver recovery.
Oil Bath (Sneha Snana)
What it is: Full-body immersion or application of warm medicated oil followed by rest.
- Rejuvenative — deeply nourishes tissues, reduces systemic dryness, and improves patient comfort and sleep during recovery.
- Used in the post-detox rejuvenation phase to consolidate gains and improve metabolic balance.
Basti (Medicated Enema) — Supportive
What it is: Medicated oil or decoction administered rectally to pacify Vata and support systemic detox.
- May be included as part of a balanced Panchakarma program when Vata imbalance coexists or when systemic nourishment is required.
- Supports digestion and systemic elimination, indirectly helping metabolic balance relevant to liver recovery.
Shirodhara (Adjunct for Stress & Sleep)
What it is: Continuous pouring of warm oil over the forehead to calm the nervous system.
- Reduces stress and improves sleep — important because chronic stress worsens metabolic and inflammatory liver conditions.
- Useful as an adjunct during preparatory or rejuvenation phases.
Herbal & Nutritional Support
Ayurvedic herbal support focuses on hepatoprotective, anti-inflammatory and digestion-enhancing herbs. All internal prescriptions are individualized and coordinated with medical treatments when needed.
- Guduchi (Tinospora cordifolia): Hepatoprotective, immune-modulating and supports liver function.
- Kalmegh / Andrographis: Traditional herb for liver inflammation and detoxification.
- Bhumyamalaki (Phyllanthus niruri): Supports liver cell regeneration and bile flow.
- Turmeric (Curcuma longa): Anti-inflammatory and antioxidant—supports liver health with piperine for absorption when indicated.
- Cholagogue & digestive aides: Trikatu/Digestive bitters to improve Agni and fat metabolism.
- Dietary guidance: Low-fat, low-refined-carb plan, avoid alcohol, reduce processed foods; small frequent meals, warm foods and adequate hydration.
If labs/imaging indicate advanced disease (fibrosis/cirrhosis), we prioritize medical coordination and supportive Ayurvedic care rather than aggressive detox.
Sample Clinic Treatment Plans
These are sample approaches — every plan is tailored after medical assessment, lab review and patient health status.
Conservative Support (4–6 weeks)- Diet & lifestyle correction + herbal program (Guduchi, Bhumyamalaki)
- Abhyanga + targeted Swedana (2–3x week)
- Monitoring labs every 2–4 weeks
Detox-directed (after clearance) — Virechana cycle- Preparatory Snehapana + Swedana → Virechana under supervision
- Post-purgation Njavara Kizhi / Oil Bath for rejuvenation
- Follow-up labs & imaging to track response
Maintenance & Prevention- Monthly supportive Abhyanga / Oil Bath
- Herbal hepatoprotective supplements and dietary plan
- Periodic monitoring and lifestyle counseling
Virechana is performed only after medical clearance and if patient is a suitable candidate. It is not done in acute hepatitis, decompensated liver disease, sepsis or major coagulopathy.

Evidence, Safety & Medical Coordination
- Some Ayurvedic herbs have hepatoprotective evidence; however, liver enlargement requires medical diagnosis and monitoring.
- Certain herbal/mineral formulations and detox procedures can stress the liver or interact with drugs — medical coordination and baseline tests are mandatory.
- We do not perform aggressive Panchakarma (Virechana) in acute hepatitis, decompensated cirrhosis, sepsis, pregnancy, or severe coagulopathy.
