What is Liver Enlargement (Hepatomegaly)?

Liver enlargement (hepatomegaly) is an increase in liver size caused by many possible conditions — fatty liver (steatosis), inflammation (hepatitis), congestion, infections, storage disorders, or space-occupying lesions. Symptoms can include right upper abdominal discomfort, fullness, fatigue and abnormal liver tests.

Common causes (modern)

  • Non-alcoholic fatty liver disease (NAFLD) / alcoholic liver disease
  • Viral hepatitis, metabolic or storage diseases
  • Drug-induced liver injury, congestive hepatopathy
  • Inflammation, fibrosis/cirrhosis in advanced stages

Ayurvedic perspective

Ayurveda relates liver disorders to impaired Agni (digestive/metabolic fire) and accumulation of Ama (toxins), with Pitta imbalance affecting Rakta and Rasa. Treatment aims to correct Agni, remove Ama, pacify aggravated Pitta, and support liver tissue (Yakrit/pleeh) regeneration.

  • Prepares the body for Panchakarma (including Virechana) and reduces muscular tension and fatigue associated with liver disease.

Swedan herbal steam therapy

Swedan (Herbal Steam / Fomentation)

What it is: Whole-body or localized herbal steam to induce sweating and mobilize toxins.

  • Used as a preparatory therapy before Virechana — aids in liquefying Ama and mobilizing toxins toward the gut for elimination.
  • Improves circulation to abdominal organs and reduces congestion.

What it is: Continuous pouring of warm herbal decoction over the abdomen or upper abdomen to soothe tissues and reduce inflammation.

  • Can be used cautiously to soothe hepatic region, improve local circulation and reduce abdominal congestion.
  • Selection of decoction depends on Pitta balance and skin sensitivity; always done with medical oversight.
Njavara Kizhi rice bolus therapy

Njavara Kizhi (Nourishing Bolus Therapy)

What it is: Warm boluses of Njavara rice cooked in herbal decoction applied to the abdomen and body to nourish tissues.

  • Provides deep nutritive support to soft tissues, promotes circulation and aids gentle tissue repair.
  • Useful in the rejuvenation phase after detoxification to build vitality and support liver recovery.
Oil Bath full body immersion

Oil Bath (Sneha Snana)

What it is: Full-body immersion or application of warm medicated oil followed by rest.

  • Rejuvenative — deeply nourishes tissues, reduces systemic dryness, and improves patient comfort and sleep during recovery.
  • Used in the post-detox rejuvenation phase to consolidate gains and improve metabolic balance.
Basti medicated enema therapy

Basti (Medicated Enema) — Supportive

What it is: Medicated oil or decoction administered rectally to pacify Vata and support systemic detox.

  • May be included as part of a balanced Panchakarma program when Vata imbalance coexists or when systemic nourishment is required.
  • Supports digestion and systemic elimination, indirectly helping metabolic balance relevant to liver recovery.

What it is: Continuous pouring of warm oil over the forehead to calm the nervous system.

Herbal & Nutritional Support

Ayurvedic herbal support focuses on hepatoprotective, anti-inflammatory and digestion-enhancing herbs. All internal prescriptions are individualized and coordinated with medical treatments when needed.

  • Guduchi (Tinospora cordifolia): Hepatoprotective, immune-modulating and supports liver function.
  • Kalmegh / Andrographis: Traditional herb for liver inflammation and detoxification.
  • Bhumyamalaki (Phyllanthus niruri): Supports liver cell regeneration and bile flow.

Virechana is performed only after medical clearance and if patient is a suitable candidate. It is not done in acute hepatitis, decompensated liver disease, sepsis or major coagulopathy.
